What are some common causes of chronic knee pain?

Hip discomfort can stem from various causes, including:

  • Osteoarthritis: This is a degenerative joint condition that leads to the wearing away of the protective cartilage within the hip joint.
  • Avascular Necrosis: There is a lack of blood supply to the hip joint, resulting in bone death.
  • Hip Fractures: These are breaks or cracks in the hip bone, frequently resulting from falls or impacts.
  • Bursitis: This condition involves the inflammation of the fluid-filled sacs (bursae) that provide cushioning for the hip joint.
  • Tendonitis: Inflammation of the tendons around the hip joint, often due to overuse or repetitive movements.
  • Hip Labral Tears: These are tears in the cartilage ring (labrum) that surround the hip socket, commonly observed in athletes or individuals with structural abnormalities of the hip.

Whether due to an injury, arthritis, or a combination of both, neglecting any form of hip dysfunction can heighten the risk of sustaining injuries in other areas of the body, given the highly interconnected nature of our musculoskeletal system.

Also known as total hip arthroplasty, hip replacement surgery may be considered if hip pain disrupts daily activities and non-surgical treatments have either been ineffective or ceased to provide relief. The most frequent cause for requiring hip replacement is damage from arthritis.

Patients typically remain in hospital for 4 days post-surgery using conventional surgical techniques. For patients who choose ERAS Direct Anterior Approach (DAA) Total Hip Replacement, the hospital stay is usually only one day. Once the anaesthetic has worn off, patients are encouraged to get out of bed and walk. This action reduces their risk of potential post-surgical complications (such as blood clot forming in calf), as well as enabling them to be discharged sooner and reducing their total hospital bill.

The Enhanced Recovery After Surgery (ERAS) approach represents a multidisciplinary, evidence-based perioperative care pathway designed to diminish surgical stress and hasten patient recovery, in contrast to traditional surgical methods. 

This approach encompasses three critical phases: preoperative, intraoperative, and postoperative care.

  • Pre-surgical counselling to ensure readiness, covering assessments like lab tests and x-rays, and potentially financial advice.
  • Advising cessation of smoking and alcohol consumption four weeks before surgery.
  • Addressing and managing any existing anaemia.
  • Providing nutritional advice, and encouraging a diet high in carbohydrates.
  • Recommending the intake of clear fluids (such as water, apple juice, or clear tea) up to 2 hours before surgery, with a solid food fast commencing 6 hours before.

During surgery, the ERAS strategy employs minimally invasive techniques to minimise tissue damage, leading to less pain and a faster recovery compared to traditional practices. This approach significantly alleviates post-operative discomfort and accelerates recovery. 

Post-surgery, a comprehensive programme is introduced to support the patient’s recuperation, marking a departure from conventional postoperative practices.
